The Role 

We are currently recruiting for a Family Practitioner to work with one of our clients in a 6 to 12 month locum. The locum is considered casual, however the role will consist of full-time hours and generally span from Monday to Friday. Flexibility is key in this role as start and end times can slightly vary depending on need.

Whilst the role is based in Ipswich, travel out to locations such as Beenleigh and Logan will be required on occasion. 

Your workplace will be primarily within the family home or other locations deemed appropriate to the delivery of the program as negotiated, some home office time is required for administration purposes.

You will support implementation of the journey plan that includes key goals and activities to support children, young people and their families achieve the desired outcome, within the home environment and community.

 

  • Contribute and promote the professional culture of the organization, treat people with respect and without judgment. Be an ambassador for change and champion the organization at all times, but not limited to, service users, contractors, and stakeholders.
  • Communicate effectively, upholding standards, and striving for excellence. Actively participate in key meetings relative to your cases and other relevant forums, as when required.
  • Adhere to policy and procedures to ensure safe and ethical practice and risk minimization. Reporting all incidents and risks in line with expectations of the role and policy
  • Be the key connection for the Department, Stakeholders, and service users, liaise with Service Coordinator to maintain quality management and risk mitigation measures.
  • Demonstrate, map and drive professional ethical practice and sound decision making through the common assessment framework (CAF), risk and incident reporting, progressive case coordination, and direction in all interactions within the case management principles.
  • Develop, implement, and maintain plans for all service users, in consideration of relevant frameworks for practice, program guidelines, and legislation requirements. Engage clients in a range of therapeutic, trauma recovery, parenting programs, intervention tools, and community resources.
  • Deliver creative case management principles in line with individual expertise and skills.
  • Be the Case Manager across the allocated cases, a small number of face-to-face hours with the client is required, other implemented with be via guidance of family support workers and driving case direction.
  • Effectively record, report and communicate all case management processes and stages relating to the clients.

To Be Successful

  • Completed Bachelor Degree qualification in a Human Service related field is highly desirable or a completed Diploma in a Human Service Related field with over 2 years of proven experience in social work, psychology, behavioural science or related discipline, Family Intervention, Family Therapy or counselling.
  • A formal qualification and evidential experience of therapeutic practice or relevant therapy.
  • Previous experience in a case management support role befit of human/community services requirements. Experience in a professional or therapeutic role working directly with complex families and young people 
  • Must be willing to work directly with families and also administration time in home office location or team meeting location (local)

  • Drivers License Class ‘C’ (Open)
  • Current Blue Card
  • National police check completed within the last 12 months
  • Current First Aid and CRP Certificate
  • A vehicle with full Comprehensive Car Insurance

 

Salary and Benefits

As a locum with ProCare, you will be under our casual rate of $53.85 per hour (base rate 6am – 8pm), $75.39 per hour (Saturday rate), $96.93 per hour (Sunday rate) + Super.
